Practical guide
A clear family care handoff checklist
A useful handoff explains the job, the evidence behind it, and what to record afterward. It should be short enough to use before a call and clear enough that two people do not unknowingly do the same work.

Name one owner and one next step
- Who is responsible for this action.
- What they need to do next.
- Why the action matters to the administrative case.
- When it is due, if the family chose a due date.
- What “done” looks like.
Include only the context they need
Point to the relevant source page or call note. Include the contact channel and any reference number only in the private care space, never in a public message or calendar title.
Say what should be recorded afterward
- Whether contact was made.
- What the outside party said it would do.
- Any response date that was explicitly stated.
- Which new source or confirmation was received.
- The next family action, or that no follow-up remains.
Check the boundary
A handoff can organize administrative work. It does not establish legal authority, make a medical decision, or replace advice from the appropriate professional.
